Can missionaries use white-out on the visa forms or correct information already filled in?

White-out and scribbles are not permitted. You can cross-out using a single line through the error.

How do I attach my photo to the visa application?

No photo is needed.

If I need a new visa application, can I print one off online?

Do not print the application from online. If you need a new copy, please contact our office and we will email you a new application. When you receive the application via email, be sure to print double-sided.

The signature on my passport does not match the name on the passport. Is there anything I need to do?

You will need to sign all your documents to match the signature on your passport.

Can missionaries take an international trip with their passport before they go on their mission?

Yes. Missionaries should have all documents turned in to Missionary Travel at least 8-10 weeks before they enter the missionary training center. We do not collect the passport.

On the form 1401 application, do missionaries leave the questions blank for numbers that are not listed on the instruction sheet to answer?

Yes. Missionaries answer ONLY the questions listed on your instruction sheet.

The application asks for copies of driver’s license, insurance, etc, but my instruction sheet does not. Do I need to send these to you?

We only need documents required on the instruction sheet.
