Frequently Asked Questions for U.S. and Canadian Residents

Dual citizens of U.S. and Japan should send a scanned copy of both their U.S. and Japan passport ID pages to jennykeyes@ChurchofJesusChrist.org. A visa is not required for bona fide dual citizens of Japan and the U.S.

If you are a dual citizen of another country, use your U.S. passport only.

Visa Application

All requested information must be completed in BLACK INK ONLY. If anything is done in blue ink, another application will need to be sent (emailed application is okay).

Missionaries should simply complete ALL of the “X’s” on the form, and then sign and date as noted.

Missionaries should provide one color visa photo in missionary attire (with suit coat, please).

Missionaries should provide the actual passport.

If missionaries make a mistake, it is okay to use white-out on the application.

Please do NOT fold the application. Send it back in an 8-½ x 11 envelope.

If missionaries have some of their visa documents, can they mail some now and mail the rest later?

Missionaries should wait until they can send all the documents at the same time.

If missionaries didn’t turn in their visa documents on time, will they be delayed?

If missionaries turned them in 30 days or more before their MTC entry date, they probably won't be delayed. If missionaries turned them in less 30 days before their MTC entry date, then they might be delayed.

If missionaries didn’t sign their middle name on their passport, is that okay?

Yes, missionaries should leave it as is.

Can missionaries keep their passport for a personal vacation before entering the MTC?

Yes, up to 45 days before their missionary training center date.

How do I attach my photo to the application?

Use a glue stick to attach your photo. Tape or staples are not acceptable.
