Do missionaries need to send in their actual passport?

Yes. Your actual passport is needed in order for Missionary Travel to apply for your visa.

My signature does not match the name printed on my passport. Is that a problem?

If you did not sign your full name on the passport, then all your visa documents must be signed the way you signed your passport.

Can I make corrections to my visa forms if the information already filled in is not correct?

You may use white out tape (not liquid) to make needed changes. Do not scribble or cross things out.

How do missionaries complete the parts of the visa application that there are no instructions for?

Missionaries MUST NOT complete anything on their visa application that they are not specifically instructed to complete.

What should I do if I haven’t graduated from high school yet and do not have my diploma?

You can wait to send in your diploma once you receive it. Please submit all other documents and include a note that you are waiting to submit your diploma until after graduation.

What do I do for the religious history requirement?

Write out a simple explanation of your history and experiences with religion and the Church. This can be one paragraph to one page long, and it needs to be signed by the missionary.
